Waivers are required by all participants every time they visit Hollywood Sports Park. A parent of guardian must fill out the waiver of any players under the age of 18 years old. All players under 18 must have their waiver notarized if they do not have a parent or guardian fill out their safety waiver in person.
This waiver will help you save time when you arrive at the park. Even if you've filled out a waiver in the pass you are still required to fill out another one. Please also make sure to fill in all the information including your date of birth. If you are under 18 then please have a parent or guardian sign the waiver before you arrive at the park.
We here at Hollywood Sports stress safety and will enforce any and all rules given by referees, staff and management. If you break any of the rules then you might be required to leave the park with no refund. Please watch the safety video below.
